ALL MEDICATIONS are administered by the school nurse during school hours. Students will need completed:

• RPS Medication Administration form with parental consent and doctor’s order
• Medication(s) delivered to school by an adult in original packaging, labeled
• Allergy Action Plan, Asthma Action Plan, & Emergency Care Plan if applicable

Metered-dose asthma inhalers, insulin and Epi-pens are the only medications that may be self-carried and self-administered by students with appropriate completed forms.

Students are not permitted to carry or self-administer ANY over the counter medication.
